We encourage every patient to establish a portal account.  This will help you save time and allow us to serve you better.

Your portal account is typically set up when you first schedule an appointment.  If you have a portal account before April of 2020, please contact us to obtain a new one.

This portal can be used to:

  • Review and verify your contact information
  • Input past medical history/medications/allergies/social history/family history/preferred pharmacy
  • View finalized visit notes and patient handouts
  • Communicate with the staff via mail or telemedicine visits.

Patient portal account can be accessed through the app PocketPatient on your mobile devices or online here

The user ID and password are set up on your first appointment.  Please contact us if you have difficulty logging in.
